The following forms are required to complete enrollment.
Please complete these forms and submit them to the Nursery School Office by no later than July 19th, 2017.

2. Required Forms From Health and Dental Providers – due no later than August 1st, 2017.

b. Oral Health (Dental Provider) Assessment Form (for children 3 and older ONLY) – click here for instructions on completing this form
3. Additional Required Forms – please print, complete, and return to the Nursery School Office by no later than July 19th, 2017
a. Registration Record for Child Receiving Care Away From Home
b. Authorization for Medical Treatment
4. Special Forms (available if medically necessary)
a. Allergy Care Plan (if your child has a known allergy)
b. Food Allergy Posting Information (if your child has a known allergy)
c. Permission to Administer Information (for any child receiving medicine while at school – requires doctor’s signature)
